You agree that you will not UPLOAD TO CASECOD.ES any patient HEALTH information or patient identifiers, e.g., name, birthdate, gender, medical record number, patient address, or billing information.  If patient identifiers are UPLOADED, we will delete the record, and reserve the right to terminate your access and use of the application.

You agree that all information you submit to CASECOD.ES WIll be accurate, complete, and truthful.

CASECOD.ES is an online service.   Upon registration, you are granted a personal, revocable, limited, non-exclusive, non-transferable license to access and use CASECOD.ES for your personal and noncommercial use in connection with an agreed upon Fellowship Program and for no other purpose.  You agree to not access, reproduce, duplicate, copy, sublicense, sell, re-sell, modify, distribute, transmit, or otherwise exploit CASECOD.ES or any of its content for any purpose except for your personal use and as described in the Terms of Use without the prior express written consent of CASECOD.ES.

You may not download, copy or use any data accessible via CASECOD.ES except the data you (if you are a Fellow) or your Fellows (if you are a Program Director) have uploaded to CASECOD.ES.  You may not modify, alter, or download any portion of the ASOPRSCoder unless specifically permitted herein or you have obtained written authorization in advance from CASECOD.ES.  CASECOD.ES reserves the right to bar, restrict or suspend any User’s access to the Website and/or to terminate this license at any time.  CASECOD.ES reserves all rights not explicitly granted in these Terms of Use.

Usernames, passwords or any other information used as part of our security procedures must be treated as confidential and must not be disclosed to any other person or entity.  Fellows and Program Directors may access CASECOD.ES to update and/or review their own records.  You are responsible for taking all reasonable steps to ensure that no unauthorized person shall have access to your CASECOD.ES account, username or password and agree to immediately notify CASECOD.ES of any unauthorized access.  CASECOD.ES has the right to disable any username, password, or other identifier, whether chosen by User or provided by CASECOD.ES, at any time if, in CASECOD.ES’ opinion, these Terms of Use were violated.

The data you submit will be maintained confidentially but may be accessed by your Program Director, by CASECOD.ES and by third-parties authorized by CASECOD.ES for purposes related to operation and administration of CASECOD.ES, and for compliance oversight.  Summary data and other information about programs, institutions, Fellows, Program Directors, or resident physician education which is not identifiable by person or organization may be aggregated and published by CASECOD.ES, or authorized for publication by CASECOD.ES in any manner CASECOD.ES deems appropriate to further the quality of the CASECOD.ES Fellowship Program, to benefit the field, and otherwise as consistent with CASECOD.ES policies and the law.  As a condition of inputting any data to CASECOD.ES (“Data”), you grant to CASECOD.ES a royalty free, perpetual, nonexclusive, transferable license to use, reproduce, copy, sublicense, adapt, modify, merge, distribute, publicly display, create derivative works from and incorporate such Data into other works together with all rights necessary to prohibit the subsequent aggregation, display, copying, duplication, reproduction, distribution, or exploitation of your Data by any other party. 

Notices.  By using CASECOD.ES you consent to receive electronic communications from CASECOD.ES (via email), and you agree that any such communications satisfy any legal requirement to make such communications in writing.  You also agree that CASECOD.ES may communicate any notices to you under these Terms of Use through regular mail, or by posting the notices on CASECOD.ES .  All notices to CASECOD.ES will be provided by sending an email to CASECOD.ES via the Contact Us email link.
